The roles will be based within Social Services, who are looking for Adults Social Care Consultant Practitioner to start as soon as possible on a full-time basis. The pay rate for this role is up to £43 per hour

Consultant Practitioner, part of Adult Social Care based at St Mary's Hospital, Newport, Isle of Wight Qualified Social Worker to join a busy team working as part of the Multi-Disciplinary Team and working collaboratively with health partners facilitating safe and timely discharges from hospital to the patient's next destination. The IOW "Care Close to Home" strategy is a vision that we have embraced which ensures that "Home" is always the first option for many patients coming out of hospital and preventing unnecessary delays to their journey. We are seeking to appoint a qualified Consultant Practitioner who will shape the practice of hospital social work provision with the support of the Group Manager. You will directly manage and supervise the team and report to the Group Manager. You will be required to work a 7 days pattern on a rota basis. The ideal candidate will have a good understanding of the role and the complexity of the work undertaken to achieve the best outcomes for people who are being assessed under the Care Act 2014 and the Mental Capacity Act 2005. You will undertake the day to day management of social work practice within the team, including the development of team members' skills and service delivery to ensure a high quality of practice that promotes independence, recovery, choice and autonomy. * Clear experience and operational knowledge of managing work load in a busy and pressurised front line team * Be able to follow a transformation plan and offer positive support to the team through this * To be able to accommodate change and lead a team in challenging circumstances * To have a breadth of knowledge and experience in supervising and supporting complex case work * Have a clear understanding of and experience working with Adult Social Care statutory duties under the Care Act 2014, Mental Health Act, Mental Capacity Act and other primary legislation and guidance including Continuing Health care. * Be able to offer clear and concise direction for team members about the above legal frameworks/guidance * To demonstrate person centred working * To demonstrate safe working practices and Risk Management but not be adverse to risk * To be able to understand the importance of data collection and interpret this * To be able to quality assure work flow to a high standard and positively challenge things that need to be improved with member of staff or teams
