Maxxima's Nursing Team are currently recruiting for a Band 3 Health Care Assistant to fill a full-time role in Chelsea, London. The ideal candidate will have an understanding of mental health practices.

Your role will include working with allocated case managers and supporting patients to engage with community health, social and recovery focused activities and services. You will develop and practice essential daily living and community focused skills, whilst also helping patients identify and work towards personal goals. You will also participate in patient related activities such as discharge and community engagement groups.

This role will be working Monday to Friday, 37.5 hours a week. The pay rate for this Band 3 Health Care Assistant is £12.00 per hour (Umbrella).

Requirements of this Band 3 Health Care Assistant vacancy:
* Must have previous experience of working with mental health clients in the community
* Must have some previous NHS experience
* Must be confident in lone working
* Must be a car driver with access to your own vehicle
