Maxxima’s nursing team is currently recruiting for a Band 6 Mental Health Practitioner/ Care Co-ordinator, who is a Mental Health Nurse or Social Worker, to fill a full-time role in the Mental Health Team in Rotherham. The ideal candidate will have knowledge and skills working in a busy community mental health service.
The post holder will work as a member of a multidisciplinary within the Adult Community Mental Health Team, providing a service to clients experiencing mental health psychotic symptoms of a serious and enduring nature.
You will be responsible as the Care Coordinator for the assessment and co-ordination of care needs, planning and implementation of evidence based care to a defined group of service users and their carers and provide professional advice and support to other agencies and members of the multidisciplinary team.
The pay rate for this Band 6 Mental Health Practitioner is £23.00 per hour (Umbrella) and is to start as soon as possible. This role is for the next 6-12 months.
Requirements of this Band 6 Mental Health Practitioner vacancy:
  • You must be a Registered Mental Health Nurse with a valid NMC Pin or Qualified Social Worker with HCPC registration
  • You must have worked within a busy community mental health team  
  • You must be a car driver with your own transport
