Job description

Maxxima’s nursing team is currently recruiting for Band 6 Care Coordinator/CPN to fill a full-time role in the Mental Health Team based in Stevenage. The ideal candidate will have excellent knowledge of working with Mental Health Services.

 

We are looking for a Band 6 Care Coordinator/CPN to work for at least 3 months in Stevenage working within the Mental Health Service

 

This role is a Band 6 position and will pay up to £24 umbrella rate

  • Working 30-37.5 hrs per week
  • Saffron Ground, Stevenage, SG1 3LJ
  • DBS required
 

The post holder will work as a member of the multi-disciplinary Community Team, will be responsible for the clinical and operational day to day service delivery, providing a high-quality social care service to adults of all ages having First Episode Psychosis. The core functions are to provide assessment, care planning and care co- ordination, including care programme approach, within a framework of recovery which embraces each individual’s right to choose and control.
