Maxxima Nursing Team is currently seeking for a Band 6 Care Coordinator to fill a full time role in St Albans.  The ideal candidate will have expert knowledge and skills working within a community mental health team as a Care Coordinatorr.
 
The hours for this role are full time, Monday – Friday 9am-5pm.
 
This NHS role will require you t o provide brief assessments of referrals to the adult mental health services. You will perform desk duty and community face-to-face contact through home visits and at the community hub. You will also attend inpatient reviews and facilitate discharges.
 
The pay rate for this locum Band 6 Care Coordinator job is £24 per hour (Umbrella), and is an ongoing contract to start ASAP.
 
Requirements of the Band 6 Care Coordinator role:
  • You must be a Registered Mental Health Nurse with a valid NMC pin or Qualified Social Worker with SWE registration
  • You must have experience within a community team
  • You must have a valid driving licence and access to a vehicle
