Job description

Maxxima is currently seeking an experienced Band 6 CHC Nurse Assessor to fill a full time, 9 am - 5 pm role in High Wycombe, North West London. The ideal candidate will have an expert understanding of CHC Nursing practices.

This NHS role will require you to be responsible for the completion of care prescriptions/care plans and timely and comprehensive assessments. You will also complete the assessment tools, in conjunction with individual /family and Multidisciplinary Team (MDT). You will also be a key member of the team, along with supporting effective communication and stakeholder management, both internally and externally, and you will manage a caseload of packages of care.

The pay rate for this locum Band 6 CHC Nurse Assessor job is £23 per hour (Umbrella) and is a full-time contract (subject to extension) due to start ASAP.

Requirements of the Band 6 CHC Nurse Assessor role:

- You must be a Registered General Nurse with a current NMC pin
- You must have previous experience working as a CHC Assessor
- You must have worked in the NHS previously
- You must have knowledge in FNC and fast track applications
- You must be a car driver with a valid licence
