Maxxima is currently seeking an experienced Band 6 Community Admission and Discharge Nurse to fill a full time, 9am - 5pm, Monday-Friday, or Wednesday-Sunday role in Grays, Essex. The ideal candidate will have an expert understanding of admission and discharge practices within the NHS.

The pay rate for this locum Band 6 Community Admission and Discharge Nurse job is £24 per hour (Umbrella) for days, plus increments for weekends, and is a 3-month contract due to start ASAP.

This NHS role will require you to provide a wide range of high quality community care services to prevent hospital admission and facilitate discharge from hospital, which will enable people to lead as independent a life as possible, and have meaningful choices". The role of the job will include, providing integrated physical health with mental health colleagues to patients with co morbid physical, functional mental health and Dementia. The post will based in the Older Peoples community mental health team

Requirements of the Band 6 Community Admission and Discharge Nurse role:

- You must be a Registered General Nurse with a valid NMC pin
- You must have previous experience working within the community or discharge planning
- You must have previous experience working with older adults
- You must have previous experience working within the NHS
