Job description

Maxxima is currently searching for a Band 6 Occupational Therapist to work in a Community Home Assessment Reablement Team, in Abingdon, Oxford.

Job summary:
  •  To provide a high standard of therapy to all patients within the Home Assessment Reablement Team (HART).
  •  To work across both the acute and community ensuring the timely assessment and planning for discharge and future care/reablement for patients who are currently in hospital or at home.
  • To work as an autonomous practitioner in the assessment and treatment/care planning of a varied caseload of complex patients, ensuring we are empowering the patients to manage their conditions and maximise their potential to reach their goals. 
  • To proactively build links between the acute and community services to facilitate multidisciplinary team working and the seamless transfer of care for patients receiving our care/rehabilitation.
  • To educate other disciplines and services about the role of HART, and effectively manage any identified problems.
  • To work towards reducing length of stay and work closely with the wards and other agencies to facilitate effective, well-coordinated discharges.
  • To participate in seven day working, including any on-call rotas and be a bleep holder as required

  • Diploma or Degree in Occupational Therapy
  • HCPC registered 
  • Experience of working within a multidisaplinary team
  • EPR Training, RiO experience would be desirable
  • Car driver with access to a car
Monday- Friday 8.00-4.00pm, 37.5h.
Contract until August.
Rates £23-30ph
