Job description

Maxxima’s nursing team is currently recruiting for Band 6 CPN’s to fill a full-time role in the Mental Health Team based in Chelmsford, Essex. The ideal candidate will have excellent knowledge of working with Mental Health Services.

 

We are looking for a Band 6 CPN’s to work for at least 3 months in Essex working with the Mental Health Service

 

The pay rate for this Band 6 CPN’s role is up to £24 (umbrella rate) and is due to start as soon as possible.

 

We are looking to cover a placement for a CPN Band 6 to start ASAP for 12 weeks, Mon-Fri 9-5.

Core Clinical Function

  • Completion of specialist memory assessments for patients to consider a diagnosis of dementia. This may be held as a clinic at the Crystal Centre, in a care home or at the patient’s own home.
  • Completion of specialist assessment tools used in the memory assessment team to include GAD-7, PHQ-9 and ACE III.
  • Obtaining full collateral history from the patient /their significant others
  • Liaising with specialist services if further information / investigations are required.
  • Attending weekly multi-disciplinary meetings to discuss assessments completed and establish if the patient has a diagnosis of dementia.
  • Produce accurate and timely documentation, conduct and manage the maintenance of accurate and timely documentation / records on all service users in line with your professional code of conduct and Trust policy.
  • Conduct risk assessment of the individual and of the potential hazards in the service user’s environment
  • Report and document incidents as required
  • Provide education around the medication prescribed, side effects, actions and aids to compliance.
  • Provide practical support, guidance and education for service users, carers
  • Undertake carers assessment in line with the Trust’s key performance indicators
  • Undertake needs assessments
  • Provide comprehensive assessment, care planning, treatment and review of care
  • Understand and utilise Health and Social Care commissioned services, demonstrating an empathetic approach of Fairer Access to Care Services
