Job description

Maxxima Nursing Team is currently seeking an experienced Band 6 CPN/Care Coordinator to fill a full time, Monday - Friday, 9am - 5pm role in Grays, Essex. The ideal candidate will have an expert understanding of mental health care coordination practices within the NHS.

The pay rate for this locum Band 6 CPN/Care Coordinator job is £24 per hour (Umbrella) and is a 3-month contract due to start ASAP.

This NHS role will require you to act as the named care co-ordinator and manage the coordination of care needs, planning and implementation of evidence based care to a defined group of service users and their carers, within a defined catchment area/locality. You will carry responsibility for the assessment and advising on the management of patients presenting to the Community Recovery and Well Being Team

Requirements of the CPN/Care Coordinator role:

- You must be a Registered Mental Health Nurse with a current NMC pin
- You must have experience working within a CMHT or Recovery and Wellbeing Team
- You must have experience working within the NHS
