Maxxima are currently looking for an experienced Band 6 Physiotherapist to join a Community Discharge Service in Belfast. The aim of the Community Discharge Service is to provide a safe, effective, person-centered approach to the transition of medically fit patients from the acute hospital to their own home / or other community setting. The service works passionately towards a 'Home First' ethos, and principle that no decisions about long-term care should be made in an acute hospital environment. The Community Discharge Service offers timely therapy follow up of patients upon discharge either at home or in an interim setting; to provide ongoing assessment +/- short term care, supporting the patients' recovery whilst determining ongoing needs.

Crucial to this post will be collaborative working and onward referral following assessment, with close links to stakeholders including Unscheduled Care, Intermediate care - Reablement/Rehabilitation Services, Community Services, independent/statutory sector and community/voluntary sector.

This post offers a unique opportunity for physiotherapists to assist in shaping the development of a Discharge to Assess model in BHSCT by using their clinical/assessment skills, knowledge and experience, and by working collaboratively within community transitional services to achieve the best outcomes for our patients on discharge from hospital.

This role is to start ASAP as our current locum is leaving at the end of November to return to England and the department would like there to be a handover completed. The role is ideally full time but all hours will be considered. 3-6 month contract minimum
