Job description

Maxxima’s nursing team is currently recruiting for Band 7 Senior Mental Health Practitioner to fill a full-time role in the Mental Health Team based in Norfolk. The ideal candidate will have excellent knowledge of working with Mental Health Services.
 
We are looking for a Band 7 Senior Mental Health Practitioner to work for at least 3 months in Norfolk working with the Mental Health Service.
 
The pay rate for this Band 7 Senior Mental Health Practitioner role is up to £28 (umbrella rate) and is due to start as soon as possible.
 
We have a placement for a Band 7 Senior Mental Health Practitioner with MH experience to work within the Crisis Assessment and Intensive Support Team in Norwich, Norfolk
 
Start date - ASAP                 
Shift pattern: Monday – Sunday, 9-5pm, 8-4pm, 10-6pm. Part time considered.
 
The ideal candidate will be a Registered Nurse with mental health experience
 
Duties:
  • Provide planned person-centred care and risk assessment / screening for young people within his/her care plan
  • To be responsible for implementing first and / or urgent assessments
  • Provide ongoing evaluation of young people’s’ needs and identify suitability for potential discharge in conjunction with the Senior Nurse, through caseload management and supervision
  • Initiate and attend person centred reviews, case conferences and other meetings as required
  • To advise the Responsible Medical Officer(s) and / or health and social care professional of any change in the young person’s conditions and / or circumstances
  • To work within the multi-disciplinary team and to form working partnerships with other care providers
  • To ensure service users and / or carer(s) are enabled and empowered to make informed decisions about their care and delivery of that care, working with agencies from within and outside the multi-disciplinary team
  • To have ongoing responsibility for the review of a young people’s care within the team setting and to prioritise and disseminate such information within the team
