Maxxima's Social Work team are currently recruiting for Community Care Officers to fill a full-time job role in Nottingham

The roles will be based within Social Services, who are looking for Community Care Officers to start as soon as possible on a full-time basis. The pay rate for this role is up to £16 per hour

Requirements and details of this vacancy

We are looking for three full time Community Care Officers, to work with our Integrated Enablement team within a hospital setting. The successful candidates will be completing assessments in order to expedite hospital discharge of medically safe citizens. Community Care Officers are responsible for ensuring statutory duties under the Care Act, including the delivery of appropriate services to safeguard vulnerable adults and their families, according to their assessed needs. This includes undertaking all levels of assessments, creating and implementing care plans and support services, and case management. Community Care Officers are also expected to deal with risk management on a daily basis, under the supervision of a senior member of the team. The successful candidates will be responsible for completing assessments of citizens in community rehab/assessment beds following hospital admission. We are moving with our system partners to a Discharge to Assess model, where no assessments will be undertaken in an acute bed.
