Hospital Social Workers – Blackburn– Hospital Services
Location: Blackburn
Job Type: Temporary/Full Time 

Maxxima are currently recruiting for Hospital Social Workers to fill a Temporary/Full Time role in Worcestershire. The ideal candidate will have an excellent understanding of Adults Services
This role is Monday to Friday, working 37 hours per week
The pay rate for this Hospital Social Workers role is up to £25 per hour umbrella. 
Intermediate Tier/Hospital social worker

You will enjoy working alongside adults to provide short intervention, goal setting and positive outcomes for people through a Discharge to Assess Pathway which supports patient journey out of hospital, back to their own home and connect them back into their communities through our neighbourhood model.
We are looking for passion, drive and motivation with the right behaviours, core values & attributes. Excellent communication is essential as is an ability to prioritise and manage your own caseload, having good organisational skills, resilience and flexibility.
You will;
- Work within a multi-disciplinary, multi-professional team to effectively establish needs and identifying options / solutions or signposting.
- Ensure that people are receiving the right support, in the right place and at the right time. Avoiding delay in patient recovery and enabling service capacity.
- Implement a strengths and asset based model of assessment and integrated health and social intervention and support planning enabling independence.
- Undertake and support staff through robust decision making. Respond to, provide and improve services by identifying issues and high risk around complex, safeguarding and crisis situations.
This role will not be without some challenges and you will need to be committed to safeguarding the welfare of the most vulnerable members of the community to enable them to lead fulfilling lives and achieve their full potential so the capacity to assess risk will be one of your strengths.
