CATCH Team (Central allocation team for care and health)  is an integrated team of health and social care professionals consisting of Occupational therapists, Physiotherapists, Technical Instructors Social workers and care coordinators.

CATCH aims to provide timely access to the most appropriate  services for patients in the community, supporting timely discharges from the acute sector in line with the home first principles and also preventing avoidable admissions to hospital through triage and deployment of support services from community health and social care colleagues.

CATCH covers the demographics of Central Lancashire.

The role will be primarily to support triage for both hospital discharge and admission avoidance but there will also be opportunities to support the wider CATCH team including direct patient facing work on the home first team completing assessments of patients in their own homes on the day of discharge from acute hospital, to assess mobility, function and social care needs to promote recovery in own environment.
