Maxxima’s nursing team is currently recruiting for Band 6 Community CPN’s to fill a full-time role in the Older Peoples Mental Health Service Team based in the London Borough of Ealing, the London Borough of Hounslow and the London Borough of Hammersmith and Fulham. The ideal candidate will have excellent knowledge of working with Mental Health Services.

 

We are looking for a Band 6 Community CPN’s to work for at least 3 months in the London Borough of Ealing, the London Borough of Hounslow and the London Borough of Hammersmith and Fulham. Working with the Mental Health Service

 

The pay rate for this Band 6 is £23 (Umbrella rate) and is due to start as soon as possible.

 

  • Daily hours of work – 7.5Hrs
 

We are looking for committed and enthusiastic nurses to join our Cognitive Impairment and Dementia Specialist (CIDS) which provides community-based treatment and care to all adults with dementia. The role of Community Nurse is to work in partnership with patients and their families/carers in the Older People Community Mental Health Service, developing their personalised care plans to enable them to lead fulfilling lives.

 

  • You will receive monthly supervision to support your development and practice.
 

  • Your role within the team would be to lead the delivery of care including education, care plans and discharge planning, for patients with complex health needs, working closely with carers, families and partner agencies to facilitate this and enable them to lead fulfilling lives.  Candidates will need to demonstrate understanding of the needs of older people with dementia and physical health care challenges.
 

  • You will be working within a friendly, well established integrated team and liaise with general health and social care colleagues to provide a service to a wide range of older people with mental illness. This will be an excellent opportunity for you to develop your skills in this area.  
 

  • You will receive superb training and development opportunities.  Working with this client group can be challenging and requires someone who is able to employ reflection and evidence based learning to enhance their own and the wider team’s knowledge and skills.
