Maxxima's Social Work team are currently recruiting for Qualified Social Workers to fill job roles in Northamptonshire

The roles will be based within an Adults Team, who are looking for Qualified Social Workers to start as soon as possible on a full-time basis. The pay rate for this role is up to £28 per hour, and the role will initially be a three-month contract, with the likelihood of extension.

We are looking for experienced Adult Social Workers with experience of working in a busy Statutory service

The Hospital based staff will be assessing, completing associated referral paperwork and will be facilitating discharge of complex cases form an acute hospital.

This will also include working in partnership with health colleagues and supporting Ward rounds, Tracking meetings and Delayed transfer of care reporting to manager. Role is based on PACE. - assess quickly to facilitate safe discharge as soon as possible. There will be covering Eligibility / social acre contributions and also discussing Discharge to assess model with families.

The community posts will be doing full eligibility assessments and finance discussions with customers who were discharged from hospital to a "discharge to assess" residential care home. This will involve full eligibility assessments / finance and all associated work (Best interest meetings / capacity assessments etc) to plan for discharge to permanent solution to meet Identified care needs. PACE will still be a high element as until the DTA bed is emptied another hospital discharge cannot enter.

Must have previous adult social care experience, within a high pace hospital environment, be able to drive and have access to a car

Flexible to work in hospital and or community environments. Hospitals are the two specific sites. Community role could travel anywhere across Northamptonshire.

NASS will not be paying any parking charges incurred as all our staff have to fund that directly themselves.

Experience of using Carefirst if possible
Must have DBS in place.
