Maxxima have recently partnered with one of the UK’s specialist provider of diagnostic and healthcare solutions. We are currently looking for a Band 4 Assistant Practitioner , who can provide overall support for the clinical leadership of the service. This post is based in Bradford, which is easily accessible via public transport.
About the role:
As an Assistant Practitioner, you will support a safe and timely hospital discharge of people who are medically fit, and no longer need hospital care. You will be working 37.5 hours per week, and work across 7 days between 8am-8pm.

Your main responsibilities as an Assistant Practitioner:
- Provide assessments and care provision to patients and support them to regain strengths and skills so that they can live as independently as possible
- Avoid admission for patients in A&E and Frailty departments who require intervention to allow them to be discharged home
- Reduce the time people spend in hospital when they no longer need acute care preventing hospital acquired infections and ‘deconditioning’ (the loss of strength and independence)
- Provide multidisciplinary reablement and rehabilitation plans, and if necessary short-term care and support, to help people gain and re-gain independence, preventing or reducing need for longer term care
- Qualification and experience in relation to Occupational Therapy, PT or Pharmacy disciplines
- The legal right to work in the UK
- UK license and access to a car
- Registration with a relevant governing body
